A JOURNEY OF RESILIENCE: DOUGLAS HARDY’S SUCCESS STORY

Reading Time: 2 minutes

At Bedrock Healthcare at Home, every patient’s story is a testament to the power of personalized care, teamwork, and determination. One such story is that of Douglas Hardy, whose incredible journey of recovery has inspired our team and beyond.

A Challenging Start

After being discharged from the hospital, Douglas faced significant challenges. He required Physical Therapy and Occupational Therapy to regain his mobility and independence. Transitioning back home was daunting, but with the support of The Florida Patient Care Therapy Group, he was determined to take on the challenge.

A Partner in Progress

Douglas was paired with Occupational Therapist Landon Francis, whose dedication and expertise proved invaluable. Together, they worked tirelessly to achieve milestones that once seemed out of reach.

When they began, Douglas needed assistance transferring in and out of his bed and wheelchair. With Landon’s guidance and a customized therapy plan, Douglas made remarkable strides. Over time, he progressed from needing help with basic movements to walking confidently with a cane.

Independence Restored

Today, Douglas can complete his activities of daily living independently—a milestone that brings him immense pride and freedom. From dressing and grooming to navigating his home with ease, he’s regained a sense of control and dignity that once felt distant.

Gratitude and Reflection

In a heartfelt review, Douglas expressed his gratitude for the care he received. The Florida Patient Care Therapy Group team was thrilled to hear from him and celebrate his success. Stories like his remind us why we do what we do—helping patients reclaim their lives and thrive in their own homes.

Empowering Recovery

Douglas’s journey is a powerful reminder that recovery is possible with the right care and support. At Bedrock Healthcare at Home, we’re proud to stand alongside our patients every step of the way, celebrating their victories and empowering them to achieve their goals.

THYROID AWARENESS: PREVENTION, MANAGEMENT AND SUPPORT

THYROID AWARENESS: PREVENTION, MANAGEMENT AND SUPPORT

Reading Time: 2 minutes

The thyroid, a small butterfly-shaped gland in your neck, plays a critical role in regulating your body’s metabolism, energy production, and overall health. Despite its importance, thyroid disorders are often overlooked or misdiagnosed, affecting millions of people worldwide. January is Thyroid Awareness Month, a perfect time to highlight prevention strategies, key statistics, and how Bedrock Healthcare at Home’s Wellness Ecosystem can support thyroid health.

Understanding Thyroid Disorders

Thyroid disorders are common but can significantly impact quality of life if left untreated. The two primary conditions include:

  1. Hypothyroidism: An underactive thyroid that slows down bodily functions, causing fatigue, weight gain, depression, and cold intolerance.
  2. Hyperthyroidism: An overactive thyroid that speeds up metabolism, leading to symptoms like rapid heartbeat, weight loss, and anxiety.

Other conditions include thyroid nodules, goiter, and thyroid cancer. Women are five to eight times more likely to experience thyroid issues than men, and the risk increases with age.

Prevention and Awareness Statistics

  • Prevalence: According to the American Thyroid Association (ATA), an estimated 20 million Americans have some form of thyroid disease, but up to 60% are unaware of their condition.
  • Screening: Early detection is crucial. Blood tests measuring TSH (thyroid-stimulating hormone) and T4 levels can identify thyroid dysfunction before symptoms become severe.
  • Risk Factors: Family history, autoimmune conditions, iodine deficiency, and exposure to radiation increase the likelihood of thyroid disorders.
  • Lifestyle Factors: Poor diet, high stress, and lack of exercise can exacerbate thyroid dysfunction. Integrating healthy habits reduces risks and improves outcomes.

How Bedrock Healthcare at Home Can Help

At Bedrock Healthcare at Home, we recognize that managing thyroid health requires a holistic approach. Our Wellness Ecosystem is designed to support prevention, early detection, and effective management through:

  1. Comprehensive Wellness Assessments – Our clinicians conduct in-depth evaluations to identify potential thyroid issues early. Regular monitoring of TSH, T3, and T4 levels is integrated into personalized wellness plans.
  1. Nutrition and Lifestyle Coaching – Proper nutrition is vital for thyroid health. Our registered dietitians provide guidance on:
  • Incorporating iodine-rich foods like fish, dairy, and iodized salt.
  • Managing autoimmune triggers through anti-inflammatory diets.
  • Supporting overall metabolic health with balanced meals and regular exercise.
  1. Advanced Remote Monitoring – Our Remote Patient Monitoring (RPM) technology ensures continuous tracking of vital signs and symptoms. Patients can:
  • Share updates with their care team in real-time.
  • Receive alerts for potential issues, enabling proactive intervention.
  1. Telehealth Access – Our Telehealth platform connects patients with endocrinologists and other specialists for:
  • Virtual consultations.
  • Medication management, including adjustments to thyroid hormone replacement therapy.
  • Guidance on managing symptoms and side effects.
  1. Integrated Behavioral Health Support – Thyroid disorders often impact mental health, contributing to anxiety, depression, or mood swings. Our Behavioral Health Integration (BHI) services address emotional well-being, providing therapy and counseling tailored to individual needs.
  1. Education and Empowerment – We empower patients with knowledge about thyroid health, encouraging:
  • Regular self-checks for thyroid lumps or swelling.

ENHANCING PATIENT OUTCOMES THROUGH COLLABORATION

ENHANCING PATIENT OUTCOMES THROUGH COLLABORATION

Reading Time: 3 minutes

For healthcare providers, collaboration is the cornerstone of success, especially when it comes to delivering high-quality, personalized care. One of the strongest examples of this is the partnership between healthcare organizations and Bedrock Healthcare At Home. Through the Bedrock Wellness Ecosystem and the Bedrock Care Management Program, Bedrock brings a holistic approach to patient care in long-term care facilities, assisted living, and other clinical centers. This collaboration not only enhances patient outcomes but also emphasizes the value of teamwork, communication, and the willingness to adjust behaviors to meet mutual goals.

How Bedrock’s Partnership Approach Drives Positive Patient Outcomes

The Bedrock Wellness Ecosystem and Bedrock Care Management Program are designed to work seamlessly with other healthcare providers. Each entity within the ecosystem plays a vital role in managing patient needs, from chronic disease management to rehabilitation and mental health support. By aligning their goals with their partners, Bedrock ensures a streamlined approach that reduces redundancy, fills care gaps, and ultimately improves patient outcomes.

The results of this collaborative approach speak volumes. Research has shown that long-term care facilities and assisted living centers that incorporate integrated care programs experience:

  • A 30% decrease in hospital readmissions, according to the Journal of Geriatric Medicine, which attributes this reduction to continuous monitoring and coordinated care plans.
  • A 40% improvement in patient satisfaction scores due to the consistency and reliability of having a team of professionals working together on behalf of each patient.
  • A 25% increase in patient mobility and independence, thanks to the skilled support from therapists and caregivers who deliver personalized rehabilitation services.

These statistics underscore the importance of structured, well-coordinated partnerships, particularly for vulnerable patient populations who rely on continuity of care.

The Power of Behavior Adjustment in Partnerships

Successful partnerships require adaptability and a willingness to meet the needs of both patients and business partners. For Bedrock Healthcare At Home, this means adjusting their approach based on the unique needs of each partner facility. This flexibility has been key to creating a collaborative environment that fosters trust and shared objectives.

For instance, Bedrock’s team actively listens to the needs of the facilities they work with, allowing them to tailor care plans and interventions that align with each facility’s protocols and preferences. According to a survey by the American Psychological Association, partnerships that prioritize open communication and listening to partners’ needs are 32% more effective. By practicing this approach, Bedrock not only fosters a productive relationship but also sets the stage for facilities to feel empowered in contributing to patient outcomes.

The Bedrock Wellness Ecosystem: Comprehensive Care in Action

The Bedrock Wellness Ecosystem includes a full spectrum of services that collectively improve patient care, including:

  • Skilled Therapists: From physical and occupational therapy to speech therapy, Bedrock’s skilled therapists provide patients with the tools and guidance they need for rehabilitation, promoting recovery and independence.
  • 24/7 Call Center: Bedrock’s call center offers continuous healthcare navigation, ensuring that both patients and staff have access to the support they need at any time of day or night.
  • Accessible Physicians: Bedside and Telehealth visits from experienced physicians allow for quick and reliable medical consultations, reducing the need for emergency hospital visits.
  • Compassionate Caregivers: Bedrock’s caregivers assist with daily activities and offer emotional support, which is critical for patient comfort and quality of life.

Long-Term Benefits of a Collaborative Approach

The willingness to adjust and support a partner’s needs yields long-term benefits for both Bedrock and the facilities they partner with:

  • Higher levels of patient trust and compliance: Patients who receive coordinated care are more likely to adhere to treatment plans and trust their healthcare team.
  • Increased facility satisfaction: Facilities partnering with Bedrock report higher staff satisfaction due to the extra support and resources provided, which contributes to lower turnover rates.
  • Improved financial outcomes: Facilities working with integrated care programs like Bedrock’s see a 30% higher revenue growth rate over five years, according to a study by the Small Business Administration.

The Future of Patient Care: A Model for Collaborative Success

Through flexible partnerships, Bedrock Healthcare At Home demonstrates that collaboration is about more than sharing resources—it’s about building relationships that prioritize patient outcomes. By embracing the Bedrock Wellness Ecosystem and Care Management Program, long-term care facilities and assisted living centers can offer their residents a higher quality of care while enhancing operational efficiency.

At the core of Bedrock’s success is the ability to adapt, listen, and support their partners, recognizing that healthcare is a shared journey. Just as in any business relationship, these partnerships are built on the principle of give-and-take. When partners like Bedrock and care facilities work together to meet each other’s needs, they not only create value for each other but, most importantly, improve the lives of the patients who depend on them.

In the evolving world of healthcare, where outcomes are everything, the Bedrock approach stands as a model for how healthcare providers can create positive, lasting change—one patient at a time.

ELEVATING PATIENT CARE WITH DEDICATED CLINICIANS

ELEVATING PATIENT CARE WITH DEDICATED CLINICIANS

Reading Time: 3 minutes

At Bedrock Healthcare at Home’s Patient Care Therapy Group, we believe in the transformative impact of lifelong learning, especially for therapists who are committed to providing the highest quality of care. This commitment to growth is why we’re thrilled to have Michaela DeLiberato, MS, OTR/L, LSVT, CDP, as one of our Occupational Therapists specializing in home care for older adults. Michaela’s journey in healthcare, marked by continuous professional development and specialization, exemplifies how passionate clinicians elevate patient care, offering skill, empathy, and tailored interventions.

Why Continuing Education Matters in Healthcare

For therapists, continuing education is essential to stay current on the best practices and emerging research that shape patient care. Occupational therapy, especially with aging populations, requires a nuanced understanding of both physical and mental health. Staying informed allows therapists like Michaela to provide effective care that adapts to the unique challenges of aging. Continuous learning fosters confidence and trust, giving patients the assurance that they’re in expert hands. For Michaela, who holds a BA in Social and Behavioral Sciences and a Master’s in Occupational Therapy from Seton Hall University, a commitment to lifelong learning has been her cornerstone since early in her career.

Spotlight on Michaela DeLiberato, MS, OTR/L, BCG, LSVT, CDP

Michaela’s dedication to her field is clear in her credentials and certifications, which include:

  • LSVT BIG: Specialization in Parkinson’s Disease therapy
  • Certified Dementia Practitioner (CDP): Training for working with patients with Alzheimer’s and other forms of dementia
  • Reiki & OT Level I & II: Integrating holistic practices into her therapy approach

Michaela’s most recent accomplishment is earning the prestigious AOTA Board Certification in Gerontology (BCG)—a distinction held by fewer than 1% of occupational therapists in the U.S., with only 142 OTs currently certified. This credential requires extensive experience: at least three years as an OT, 3,000 hours in geriatric care, and 500 direct patient hours. Her certification represents a milestone in her career and for Bedrock Patient Care Therapy Group. Michaela notes that “achieving AOTA’s BCG has been a goal since I started working with older adults. I’m incredibly proud to bring this level of expertise to our team and my patients.”

The Value of Specialized Clinicians in Geriatric Care

In Michaela’s words, “As the older adult population continues to age, it is imperative that we advocate for occupational therapy and the value we bring to helping older adults.” This population often faces unique challenges, from maintaining mobility to recovering independence in daily tasks. Specialized knowledge, like Michaela’s, allows clinicians to create targeted interventions that significantly impact patient lives, helping prevent hospitalizations and enabling aging in place.

Michaela’s approach centers on “thinking outside the box” to meet patients’ goals, a perspective informed by her early experiences with family members in healthcare and her longstanding passion for helping others. Raised on the Jersey Shore and influenced by her parents’ volunteer work as EMTs, Michaela found her calling in occupational therapy—a role that allows her to build relationships with patients and improve their daily lives.

Continuing Education as a Path to Quality Patient Care

Continuing education and advanced certifications, like the BCG, are crucial in today’s healthcare landscape. They are a testament to a clinician’s commitment to both their field and their patients. Michaela believes that “earning your BCG adds value to the profession by demonstrating excellence of care and critical thinking in geriatrics.” She highlights that by staying informed on the latest research, OTs can serve their patients better, providing specialized care tailored to their unique needs.

Looking Forward with Passionate, Skilled Clinicians

We’re proud to support and celebrate clinicians like Michaela, who bring expertise, compassion, and a commitment to lifelong learning to their practice. Michaela’s story serves as a powerful example of how continued professional growth benefits both the therapist and their patients, fostering a healthcare culture that champions patient well-being, independence, and fulfillment.
