I am very happy with the care that the providers with PCMG are currently giving to my father-in-law. He was at another facility, under the care of a different team, and we had to have him moved. The transitional care navigators were so helpful in getting him transferred to a facility where the care is more comprehensive and gives us a level of comfort that he is being properly cared for.
